Rating Application and Appeal Forms

If you would like to submit an application for a West Florida PHRF rating, please print both pages of the Application Form, fill out with as much information as possible and return with your check to:

The fee for an new rating application is $35.00 for US Sailing members and $40.00 for non US Sailing members. If you're not currently a US Sailing member, it's very easy to join.
We will usually be able to process your application within a week. Please be sure to include your contact phone numbers and clearly print your email address in case of any questions.
